Subcutaneous rheumatoid nodules which occur most commonly in rheumatoid arthritis show central draining perforation rarely.
We report a cases of perforating rheumatoid nodule occuring on the occiput of a patient who had a history of rheumatoid arthritis for 3 years. A biopsy specimen of the subcutaneous nodule with central core on the occiput showed extensive
eosinophilic or necrobiotic basophilic materials replacing the papillary dermis to the base f the specimen. A column of necrobiotic, PAS-positive material extended through the overlying dermis and epidermis to the skin surface.
